A Nursing Home that’s Person-Centered Every person is different. At Cabrini nursing homes, we provide a nurturing and safe environment and develop a personalized plan of care for each person, whether as a long-term resident, a short-term patient or community resident receiving respite care. Skilled nursing Skilled nursing is at the center of the care we provide. Nursing Care Coordinators lead a team of nurses and certified nursing assistants to carry out the resident’s plan of care. Residents receive all the professional and medical services they require, including regular visits by board-certified internists and geriatricians to address their changing medical care needs. A physician may also order rehabilitation services based upon the individual’s medical condition; ongoing rehabilitation assessments are made throughout the resident’s stay. Specialty care and services A complete array of specialist services is available to meet the individual needs of each resident. Services cover the complete spectrum of needs, from audiology to wound care, and are delivered onsite by licensed and Board Certified professionals sensitive to the special needs of older adults. Social Workers, Pastoral Care Workers, Clinical Psychologists, and Psychiatrists are also available to address the emotional issues which often accompany physical illness and disability in the elderly. Hospice care Hospice care in the nursing home is provided by a specialized team of professionals who work to enhance and supplement the care and services, allowing residents in the advanced stages of illness to remain in familiar and supportive surroundings. Hospice care enhances the expertise of the nursing home team with the special care, emotional support and guidance residents and their families need during this distressing time. Part of a community We believe that a creative use of time for our residents engenders a renewed sense of social belonging and interaction, as well as an enhanced sense of well-being. From the day of admission, we strive to make our residents feel at home, whether through our food preparation, helping a resident to pursue a favorite hobby, or through memory-evoking activities such as music and storytelling. |

Short Term Rehabilitation/Sub Acute Care This program is designed for patients who are ready to leave the hospital but require additional care before returning home. Our short-term rehabilitation/sub-acute care program is suited for the patient recovering from a stroke or a fracture, or an extended hospital stay. The services provided by Cabrini’s interdisciplinary care team help former hospital patients to return to independent living. Whether the stay is as short as a week or as long as several months, the patient receives a personalized plan of care that assures appropriate medical, therapeutic, and social services at every stage of rehabilitation. Services provided in the short-term rehabilitation/sub-acute care program are: Physical and occupational therapy Speech language therapy Wound care Intravenous therapy
