The goal of our Long Term Home Health Care Program (LTHHCP) is to provide each patient with a comprehensive, coordinated program of nursing-home-level care in his or her own home. The major advantage of the “nursing home without walls” program is that it enables families to remain together, while providing the large variety of services that long-term care patients require. All the essential services for meeting the long-term care needs of chronically ill or disabled individuals are provided through Cabrini’s LTHHCPs, including: |

Eligibility Chronically ill or disabled adults are eligible for Cabrini’s LTHHCP if they: are in need of preventive, restorative or supportive nursing services are under the medical supervision of a physician; and reside in Manhattan, Brooklyn, Queens or WestchesterAdditionally, the cost of needed services must not be expected to exceed 75% of the cost of comparable care provided in an institutional setting. |
