Long Term Home Health Care Program Back

The goal of our Long Term Home Health Care Program (LTHHCP) is to provide each patient with a comprehensive, coordinated program of nursing-home-level care in his or her own home. The major advantage of the “nursing home without walls” program is that it enables families to remain together, while providing the large variety of services that long-term care patients require.

All the essential services for meeting the long-term care needs of chronically ill or disabled individuals are provided through Cabrini’s LTHHCPs, including:
 Skilled Nursing  Home Health Aides
 Physical Therapy  Personal Care Workers
 Occupational Therapy  Housekeeper
 Speech Language Pathology  Respiratory Therapy
 Audiology  Laboratory Testing
 Social Worker  Medical Transportation
 Nutritional Counseling  Durable Medical Equipment & Supplies
 Personal Emergency Response System

Eligibility
Chronically ill or disabled adults are eligible for Cabrini’s LTHHCP if they:
 are in need of preventive, restorative or supportive nursing services
 are under the medical supervision of a physician;
 and reside in Manhattan, Brooklyn, Queens or Westchester

Additionally, the cost of needed services must not be expected to exceed 75% of the cost of comparable care provided in an institutional setting.
