Patients living with HIV have special needs. We developed the AIDS Home Care Program (AHCP) as a comprehensive home care program that provides an alternative to institutionalization for people living with HIV/AIDS who are chronically ill and disabled. The mission of the AHCP is to offer a complete range of in-home health and social support services. Each patient’s special needs are met through an individual plan of care developed by a Registered Nurse who is also the patient’s case manager. |
